Use this service to opt out of your summary care record.
You can use this service if you:
- are registered at the surgery
Your summary care record is a short summary of your GP medical records that includes details of any medicines you are taking, any allergies you suffer from and any bad reactions to medicines that you have had.
This means they can give you better care if you need health care away from your usual doctor’s surgery, for instance in A&E or when you’re on holiday.
It also includes your name, address, date of birth and your unique NHS number to help identify you correctly. Additional information can also be added to your summary care record.
If you agree, we will include additional information in your Summary Care Record. This will help you get better care when you are away from the GP practice.
If you do not allow us to include additional information in your Summary Care Record, your care may be harmed if you get seriously ill.
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
